Accessible Booking and Intake Forms: A Checklist

Booking and intake forms are where an accessibility problem costs a business most directly, because a customer who cannot finish the form cannot book.

An accessible online form is one that a person using a screen reader, a keyboard, voice control or a magnified screen can fill in, correct and submit without help, and the core of it is a visible label for every field, clear instructions and errors that explain how to fix them. The checklist below follows the W3C's forms tutorial and the Web Content Accessibility Guidelines (WCAG) 2.2, the version the W3C published as a Recommendation in October 2023.

The checklist

#CheckWCAG referenceHow to test it
1Every field has a visible label tied to it, not just gray placeholder text1.3.1, 3.3.2Click the label; the cursor should jump into the field
2Related choices are grouped with a heading, such as "Preferred appointment time" over a set of radio buttons1.3.1Listen with a screen reader (VoiceOver or NVDA) and check the group name is read
3Instructions come before the field, including formats ("MM/DD/YYYY") and which fields are required3.3.2Read the form top to bottom without guessing
4Errors are named in words, next to the field, with how to fix them3.3.1, 3.3.3Submit the form empty and read every message
5Color is never the only signal for an error or a required field1.4.1View the form in grayscale

11Time limits can be turned off or extended2.2.1Leave the form open for 20 minutes, then submit
12Long intake forms are split into steps with a progress indicator and a confirmation at the endW3C forms tutorialCount the steps and check the final screen confirms what was sent

What the web looks like today

Exhibit 1. Form and button problems are among the most common failures on the web Share of home pages, by failure type Missing form input labels 51% Empty links 46.3% Empty buttons 30.6%
Source: WebAIM, The WebAIM Million 2026 report; automated scan of one million home pages, February 2026.

Why it works

The WebAIM Million scan of one million home pages found missing form input labels on 51% of them in 2026, so the first item on the checklist is also the form problem found most often. Labels, grouped choices and specific error messages help every customer, not only those using assistive technology, and the W3C's tutorial notes that asking for more information than the task needs leads people to abandon forms, a problem the published guide What Every Extra Step in a Booking Form Costs looks at from the booking side. The Department of Justice's guidance on web accessibility names inaccessible forms, including those without labels or clear error messages, among the barriers businesses should fix.
